Diagnostic Reading Assessment Grades 4-6

See how students use text evidence before choosing what to teach next. This Grades 4-6 classroom diagnostic contains four original forms with 54 items, a 20-page Student Book, a 32-page Teacher Guide with keys and scoring guidance, six printable evidence cards, 27 editable PowerPoint slides, a nine-sheet Excel tracker and an offline HTML reading edition. Review the answers and supports used, choose a teaching focus, then use one of six 10-15 minute routines and revisit it with a later form. The HTML edition does not save answers. This is a classroom planning resource, not a standardized assessment. The forms differ in content and task demands; raw-score or percentage differences do not measure growth.
